Replacements for Lost Valid and Expired Epassport

Note: Clients are reminded to bring their complete sets of documents, including originals and photocopies, for their own convenience.

ØOriginal Philippine Statistics Authority (PSA) authenticated documents that will support the change of name (if applicable)
         • Marriage Contract
         • Annotated Birth Certificate
         • Annotated Marriage Contract to show annulment/divorce/court ordered instruction
         Death Certificate of Spouse

ØPhilippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper and a photocopy
ØAffidavit of loss in English
ØPolice Report in English
ØMarried Females: Original copy PSA Authenticated Marriage Contract on Security Paper
ØOriginal and copy of any of the following valid IDs :

  • Social Security System (SSS)
  • Government Service Insurance System(GSIS)
  • Unified Multi-Purpose Identification (UMID)Card
  • Land Transportation Office (LTO) Driver's License
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) card
  • Commission on Elections (COMELEC) Voter's ID or Voter's Registration Record from COMELEC Head or Regional Office
  • Philippine National Police (PNP)
  • Firearms License
  • Senior Citizen ID - Persons with Disabilities (PWD) ID 

Fees

RM 432.00 for Lost Green or MRP Passport
RM 720.00 for Lost E-Passport

The above documents should be submitted to the Embassy for processing and evaluation. An interview by a Consular Officer will be conducted. There will be 15 working day clearing period. After the clearing period, the applicant will return to the Embassy for the enrolment and data capturing process.
